Join our team as an Outreach Worker at Astoria Coastal Family Health Center! This role provides our patients with the needed educational information related to social service agencies available in the Astoria, OR area. Position Highlights:

What You'll Do:
Assess patients' current situation and identify services needed to ensure their basic needs and concerns are addressed Administer the "Social Determinants of Health" tool to facilitate disclosure of additional needs or concerns like mental health, nutrition, physical activity, and/or finances Make referrals to appropriate agencies and service providers based on identified needs Provide information and assist clients with self-referral U Equal employment opportunity, including veterans and individuals with disabilities.
