Therefore, we take a personalized approach in caring for our residents, providing comprehensive medical, emotional, and social support. We are searching for experienced, caring, & dependable applicants to join our TEAM on a part-time basis. Responsibilities Assures that notes are informative and descriptive of services provided and of residents' responses to services Prepares and completes social service documentation in timely manner Develop a social history on each resident Complete sections of the MDS Assists with addressing resident concerns and searching for a positive resolution At times, participates in the Utilization Review and other committees as directed and needed Develops discharge plans at the point of admission to provide direction for the resident's stay Qualifications BSW or MSW Knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care Previous long-term care experience is desirable.
