Full Job Description SUMMARY:
Under the supervision of the Program Supervisor, the Support Coordinator will assure program compliance according to state and federal guidelines and Agency standards. The Support Coordination program includes the targeted populations designated by the Department of Health and Hospitals, (OCDD) (OAAS) and Infant Toddler. - Remain objective while accepting the recipient's/ guardian's lifestyle A Support Coordinator is employed 40 hours per week and maintains normal business hours of 8:00 AM to 5:00 PM Monday through Fridays.
Essential Duties:
Support Coordinators will: Update schedules for the week on Outlook each Monday by 9:00 AM. Each appointment must include; date, time, location and recipient's initials for visits; date, time and location of weekly supervision; date, time and location of all meetings, trainings, personal appointments and leave time. Changes or cancellations to the posted schedule should be noted on World Client within the same week as a notation. Do not delete the original information. Maintain an assigned caseload in accordance with State guidelines with NOW, ROW, EDA, EPSDT, ADHC, Early Steps and LCC cases. (At times, with State approval, caseloads will exceed those listed in the programmatic guidelines) Schedule and complete face-to-face supervision for a minimum of one hour per week with their Supervisor. Complete 16 hours of orientation within the first five working days of employment; a minimum of eight hours of the orientation training will include specific service needs and resources of the target population. Complete an additional 16 hours of training during the first 90 calendar days of employment, if the new employee has no documented training. Report to the office daily and maintain regular business office hours when required by their Supervisor Check e-mail, phone/pager and mailbox daily and respond to all messages daily. Complete 40 hours case management related training annually Meet all timelines as required by the Department of Health and Hospitals (DHH/ OCDD/OAAS) for NOW, EPSDT, LCC, SW, EDA, ROW, ADHC, Early Steps Populations and the Agency. (CADENCE of Acadiana, Inc. may establish Agency guidelines and timelines that take precedence over DHH/OCDD/OAAS and Medicaid timelines to ensure State requirements and Agency expectations are met) Meet all minimum requirements of case management service delivery as mandated by CADENCE of Acadiana, Inc., DHH. OCDD/OAAS and Medicaid Complete and submit service logs/summaries daily for each service delivered, i.e. visit, phone call, written contact. Complete and submit quarterly summary service log (OAAS/OCDD populations) Complete revisions timely and as required by programmatic policy. Use person-centered and personal outcome approach when completing CPOC and IFSP Complete all intake, assessment and service plan documentation properly and obtain all required signatures Complete closure forms and submit to supervisor and support staff within five days of closing recipient case Send appointment notices to recipient/family and providers for initial, quarterly, six-month, annual and 90-day transition meetings according to programmatic policy. Assure services are being delivered according to CPOC or IFSP at quarterly meetings Maintain confidentiality of recipient and recipient record according toHIPPA/FERPA
policy. Use correct error correction procedure on all documentation File, shred, and clean work area weekly. This is to include file drawers, bins, floor area and refrigerated items. Provide ongoing current driver's license, automobile insurance and training information to the Human Resources Department and on-site project manager prior to expiration. Attend and actively participate in mandatory weekly Team Meeting providing opportunities for problem solving, feedback and support to case management team Attend all agency mandatory meetings and training Attend all regional Support Coordinators meetings and attend state level meetings as requested. The Support Coordinator is responsible for any identified changes to the job duties as designated and deemed necessary per changes in program policies from OCDD/OAAS, EarlySteps or Medicaid or as designated per the Director, Administrative Team or the Quality Improvement Team Maintain professional attire at all times. In an effort to promote a positive and productive work environment and in order to strive towards the vision of the Agency, each Support Coordinator is to at all times demonstrate a polite, professional, positive attitude and demeanor in the office, with all staff/families/recipients/providers/coworkers/State employee and when representing the Agency in the community. Perform other duties as deemed necessaryQUALIFICATION REQUIREMENTS

Minimum requirements will be those required by the Louisiana Medicaid Case Management Services Program for Support Coordinator as listed below. 1. A Bachelor's degree in a human service related field including but not limited to Psychology, Education, Counseling, Rehabilitation Counseling Social Services, Sociology, Philosophy, Family and Consumer Sciences, Criminal Justice, Child Development, Substance Abuse, Gerontology and Vocational Rehabilitation; a bachelor's in liberal arts or general studies with a concentration of at least 16 hours in one of the fields aforementioned. OR 2. A licensed registered nurse; AND One year of paid experience after licensure as a registered nurse in public health or a human service related field providing direct recipient services or case management/ service coordination; OR 3. A Bachelor's or Master's degree in Social Work from a social work program accredited by the Council on Social Work Education;Job Type:
