A Social or Human Services Assistant assists with identifying and arranging community services for clients. Works with social workers and other human services professionals. Helps with rehabilitation for people with disabilities or those in recovery, or coordinates home care services for the elderly; may also coordinate housing or job training assistance for immigrants, veterans, former inmates or families in crisis.

Job Description

Qualifications:
Must have a high school diploma or equivalent. Must be able to speak Spanish and English fluently and be able to translate for patients. A valid SC Driver's License is required. Will be required to travel to various community events. The Translator's interpretation skills will be evaluated yearly for accuracy. Job duties include but are not limited to: Greet all patients with a smile and interact with them in a courteous and friendly manner Maintain strict confidentiality of all protected health information Must be accessible during work hours and check voice messages throughout the day Schedule and keep track of patients' appointment Transport agricultural workers to and from their office visits Call patients to confirm they have transportation the day before their appointment Interpret for Spanish-speaking patients when scheduling an appointment, relying messages, and giving test results Refer patient complaints/ issues to the Front Office Manager or someone in Administration Distribute clothing, fliers, and other items in the community regarding upcoming events, health information, and the services offered at our offices Provide health information to agricultural workers at the camp sites Transport medication to agricultural workers who are unable to pick up their prescriptions Assist agricultural workers with understanding medication dosage and usage Represent Low Country Health Care System at community health fairs and other community events Complete, fax, and email the Migrant report "Merge" to the South Carolina Primary Health Care Association monthly Complete the Monthly Activity Report by the 1st of each month Complete the End of Season Report Complete the Needs Assessment Questionnaires (NAQ) monthly from the grower/crew leader, the community, and the agricultural workers Accurately maintain records, reports, and forms associated with their job duties Provide follow-up visits with Spanish speaking patients Meet quarterly with the South Carolina Primary Health Care Association via webinar, telephone conference, or virtual Zoom calls Provide quarterly "task" meetings for crew leaders to assess the needs of agricultural workers and their families Identify patients that are eligible for the Migrant Program and complete an eligible/intake form Distribute Migrant (MIG) cards to qualified agricultural worker Assist the transportation department as needed Ensure patients are at their appointments 15-30 minutes before their appointment time Attend training, workers, and continuing education as instructed by your supervisor Perform any other duties as assigned by your supervisor •Salary scales are a range of pay and offers are made to applicants based upon the requirements of the particular position. Applicants with "previous like skills" may not be given the same weight and consideration as "previous specific same skills": i.e., For a Medical Receptionist position, "Previous clerical skills" may not be preferred over "Previous Medical Clerical Skills"
