Build and maintain partnerships with community organizations, schools, local agencies, and other stakeholders. Coordinate and represent the organization at health fairs, community events, and outreach activities. Promote services including behavioral health, chronic disease management, women's health, pediatrics, eye care, pharmacy, and other NTACHC services. Develop culturally appropriate materials, including flyers, and campaigns. Track outreach activities, patient engagement, and campaign effectiveness; provide regular reports and insights. Support initiatives to increase new patient acquisition and retention. Ensure all marketing and outreach efforts align with FQHC regulations and organizational branding standards. Assist in identifying community needs and gaps in services through engagement with the community and feedback. Maintain outreach calendars, event logistics, and inventory of promotional materials. Support patient education related to available services and programs. Monitor key performance indicators (KPIs) such as new patients, event participation, referrals, campaign return on investment (ROI), and engagement. Assist with special projects and organizational initiatives as assigned. Adhere to HIPAA regulations and patient confidentiality standards Follow organizational policies related to data security and information Other duties as assigned. Knowledge, Skills, and Abilities Knowledge of community outreach strategies and basic marketing principles. Understanding of underserved populations and social determinants of health. Strong interpersonal and relationship-building skills. Excellent verbal and written communication skills. Ability to plan, organize, and manage multiple projects and events. Proficiency in social media platforms, marketing tools, and Microsoft Office. Ability to analyze data and measure outreach and marketing effectiveness. Cultural competency and ability to engage diverse populations. Creative thinking with attention to detail and brand consistency. Bilingual skills (preferred, depending on community needs). Minimum Qualifications Bachelor's degree in Marketing, Communications, Public Health, or a related field preferred (or equivalent experience). Minimum of 2 years of experience in community outreach, marketing, or healthcare engagement. Experience managing social media platforms and digital marketing tools. Experience in health education and community outreach. Experience working in an FQHC, community health center, or nonprofit organization preferred. Valid driver's license and ability to travel locally for outreach events. Job Summary The Community Outreach Worker is responsible for developing and executing outreach strategies that increase community awareness, drives patient growth and engagement, and utilization of services such as social media, and campaigns. This role builds strong relationships with community partners, promotes health programs, and supports the organization's mission to provide accessible, high-quality care to underserved populations. Supports growth in patient volume, enhances access to care, and aligns services with population health priorities.
