A Lead Outreach Worker contributes to AHF by re-engaging clients formerly lost to care, coordinating and implementing specific interventions to access clients who have fallen out of care, and assisting them to return to care. The Lead Outreach Worker (LOW) will utilize strong experience in understanding the locality and the specific challenges facing this population and working to find creative solutions to locate and re-engage these clients. The LOW will conduct ongoing retention meetings with healthcare center staff to ensure no gap in retention efforts and to uphold radical cooperation across divisions. By combining their knowledge of the Ryan White Care Act Program within their area, the LOW will facilitate enrollment in the Care Management program when clients are re-engaged by working closely with the Licensed Clinical Care Managers and Non-Medical Care Managers. The LOW must attend the regular Outreach-focused training meetings conducted by the Ryan White Program. The LOW will locate and contact clients compassionately to ensure successful linkage and retention. A Lead Outreach Worker enhances the reputation of AHF by working to find and re-engage clients and seamlessly engage into care services with the ultimate goal of viral suppression and retention in care. The Lead Outreach Worker will also participate in the Ryan White Harris County Planning Committee as a representative of AHF to fulfill contract requirements. The Lead Outreach Worker and clinic staff will work as partners in re-engagement and retention efforts. Beyond the scope of the contract, your work contributes to measures established by the
HIV/AIDS
Bureau (HAB) Measures and the Department of Health and Human Services. You will be a passionate advocate for our top initiatives. To be successful as a Lead Outreach Worker it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees. AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Lead Outreach Worker will work as a part of a close-knit team to facilitate new ways on how to re-engage and retain clients in care, review and investigate the prior client history, implement a plan of action, make phone calls, and visit community sites to locate clients. Re-engaging clients and retaining them in care are the goals of the LOW so that viral suppression is achieved, resulting in a healthier population. You will have an opportunity to be an advocate, enhance the reputation of AHF, and encourage clients to continue to seek treatment from an AHF Healthcare Center.
Licenses and Certifications:
Associate Degree required Valid driver license as position will require driving duties Able to travel within the service area 40% or more each week Proficient Outreach Skills knowledgeable about HIV/AIDS and current community resources. knowledge of HIV risk behaviors Experience in conducting outreach to engage individuals in HIV care.
