Job Description
Required:
College degree or three years of related experience and/or training or equivalent combination of education and experience.Excellent verbal and written communication skills.
Strong motivational and convincing power.
Ability to counsel clients of all ages and problems.
A well organized and pleasing personality.
Excellent knowledge of available social services.
Ability to understand client needs and develop client-specific engagement processes that will be most successful.
Technical writing skills.
Research skills.
Knowledge of Microsoft Office programs.
Desired:
Certification in Life Coaching from an accredited body such as the International Coaching Federation (ICF).Experience working with Medicaid, WVCHIP, or other disadvantaged populations.Responsibilities:
Provide effective solutions and suggestions on home, workplace, and community issues.Provide telephonic and/or face to face support and education to The Health Plan (THP) members.
Establish proficiency in THP programs and applications necessary to perform the functions of Life Coach in the care management platform, HEART, Healthwise, and any other relevant program(s).Counsel and advise clients to improve professional skills.
Offer possible solutions to address professional needs.
Involve clients in motivational interviewing and engagement strategies to better identify client needs.
Apply knowledge of resume development, job opportunities, and interviewing skills and techniques to assist the clients with their needs.
Assess member needs such as housing, transportation or food insecurities, and make referrals to appropriate resources.
Assist with identifying education/training opportunities and financial assistance that may be available.
Assist with understanding budgeting and finances to help promote mental wellness.
Coordinates with health coach team to help provide a holistic care plan, inclusive of both social and health wellbeing.
Deal with the client with a positive and empathetic attitude to make them feel comfortable. 8:00am - 5:00pm40
