Job Description

JAIL-BASED
Case Management in Jackson County in AL in Northeast AL The Jail-Based Care Coordinator position will support formerly incarcerated individuals as they transition them back into the community. Duties will include connecting these individuals with community and government services, housing, employment, etc. The Care Coordinator will work in tandem with the Jail-Based Therapist.
Required Qualifications:
Bachelors degree in a
HUMAN SERVICES FIELD
•(Sociology, Psychiatry, Social Work, Counseling, etc.) Those whom do not meet the above educational qualification need not apply. The degree must be completed.
Summary of Responsibilities:
This is a direct service position involving basic casework principles and extensive direct client contact. As part of the treatment team, the Care Coordinator is responsible for coordinating services for the mentally ill. The Care Coordinator is responsible for training and educating client and family regarding symptoms, treatment, and resources, and for assisting with crisis situations when needed. The amount of contact is dependent upon the individual's needs and level of dysfunction.
Examples of Job Requirements:
Participate in clinical staffings. Conduct assessment of client's strengths and needs for case management services. Develop case plan identifying clients' goals for improvement. Provide case management services as indicated in case plan. Arrange transportation for clients when needed to accomplish service plan goals. Make service referrals and arrange for client's appointments with other service providers, in accordance with service plan. Provide supportive services to clients as needed. Conduct assessment of basic living skills needs for Basic Living Skills services. Provide Basic Living Skills training, Family Support and Education services, and Crisis Intervention/Resolution services as indicated in the clinical treatment plan. Work cooperatively with therapists and other service providers to identify and contact clients in need of services. Maintain current and adequate documentation of services provided and submit billing data on a timely basis. May be given supervisory and leadership responsibility by assignment. More specific responsibilities and assignments may be specified in the individualized job performance contract. Care Coordinators will be required to drive a vehicle and, at times, climb stairs at client homes.
Required Qualifications:
Bachelor's degree in a human service field and completion of approved training program.
Supervisor:
The supervisor of this position will be a Program Director.
