Full-Time Medical Hill Healthcare Center Pay:
$80,000-$85,000 annually

Full-Time Lead with Compassion. Make a Lasting Impact. Medical Hill Healthcare Center is seeking a dedicated and experienced Social Services Director to join our leadership team. If you are passionate about advocating for residents, supporting families, and creating meaningful care experiences, we'd love to hear from you. As the Social Services Director, you will oversee the facility's social services program, ensuring residents receive the emotional, psychosocial, and discharge planning support they need to thrive throughout their stay. What You'll Do Develop, coordinate, and manage the facility's social services program. Complete psychosocial assessments and individualized care plans. Assist residents and families with adjustment, counseling, and crisis intervention. Coordinate discharge planning and facilitate safe transitions of care. Connect residents and families with community resources and support services. Participate in interdisciplinary care plan meetings. Advocate for residents' rights while ensuring compliance with state and federal regulations. Maintain accurate documentation and regulatory compliance. Collaborate with physicians, nursing staff, therapists, and outside agencies to provide exceptional resident-centered care. Qualifications Bachelor's degree in Social Work, Human Services, or a related field required (Master's preferred). Previous experience in a skilled nursing facility or healthcare setting preferred. Knowledge of Medicare, Medi-Cal, discharge planning, and long-term care regulations. Excellent communication, organization, and interpersonal skills. Ability to work collaboratively in a fast-paced environment. Current California Social Services certification (or eligibility) preferred, as required by state regulations.
