We are seeking a dedicated and detail-oriented Housekeeper to join our skilled nursing facility team.
RESPONSIBILITIES
As the Social Services Director, you will be responsible for planning, developing, organizing, implementing, evaluating, and directing social service initiatives, ensuring the medically related emotional and social needs of each resident are met and maintained on an individual basis. Lead andassistin the planning, development, organization, implementation, evaluation, and direction of the facility's social service programs. Activelyparticipatein discharge planning, and the development and implementation of social care plans and resident assessments. Conduct interviews with residents and their families as needed. MaintainaccurateandtimelySocial Service progress notes, documenting responses to treatment plans and adjustments to institutional life. Facilitate access to community social, health, and welfare agency resources to meet resident needs. Maintain consistent contact with residents' families providing progressreportsas necessary. Assistin the effective resolution of grievances voiced by residents, families, responsible parties, or ombudsman.
Qualifications:
Bachelor's in Social Worker related field (Sociology, Special Education, Rehabilitation Counseling, Psychology, etc.) Minimum of 1 year experience as a Supervisor in a Skilled Nursing Facility
