Location:
Miami, FL A Skilled Nursing Facility is seeking an experienced Social Services Director to oversee psychosocial services, discharge planning, and resident advocacy within the facility. This is a full-time, permanent leadership role requiring prior experience in a Skilled Nursing or long-term care setting. The Social Services Director will ensure compliance with federal and state regulations while supporting residents and families through admissions, care planning, and discharge transitions. Key Responsibilities Oversee all social services operations within the Skilled Nursing Facility Lead discharge planning processes and coordinate transitions with hospitals and community providers Ensure compliance with CMS and state long-term care regulations Participate in interdisciplinary care plan meetings and QAPI initiatives Address resident rights, grievances, and family concerns Maintain accurate, compliant documentation Collaborate with Admissions, Nursing, Therapy, and Administration Support survey readiness and regulatory processes Required Qualifications Minimum 2+ years of experience as a Social Worker or Social Services Director in a Skilled Nursing Facility required Bachelor's degree in Social Work, Human Services, or related field required (Master's preferred) Strong understanding of CMS regulations and long-term care compliance Experience with Medicare, Medicaid, and discharge planning processes Proven ability to manage resident and family communication in a SNF environment Hospital-only experience will not be considered.
