Licensed Professional Counselor

Responsible for planning,organizing,developing, and assisting with the overall operation of the Social Service Department for a long-term care nursing facility in compliance with federal,state,and local regulations and center guidelines. Participate in discharge planning,development and implementation of social service and needs of resident and family. Completes required forms and documents in accordance with company policy and state and/or federal regulations. Implements social service interventions that achieve treatments goals, address residents needs, link social supports, physical care and physical environment to enhance quality of life. Coordinates Family council Coordinates grievance complaint process Responsible for meeting documentation requirements for long-term care guidelines related to social services. Process ability to communicate effectively, both orally and in writing.
