Other duties may be assigned) Within 24 hours: Initiate treatment plan, complete social services portion of treatment plan, Complete a tentative discharge plan Within 72 hours: Review patient progress in treatment plan, complete psychosocial assessments on each patient Review patient progress weekly Discharge meeting with patient, and family (if requested), to discuss the discharge and crisis plan as well as patient and family expectations Advocate for patient's rights and ensure the patient's needs for services while admitted and post discharge are being offered/referred Responsible for making appropriate referrals to community resources and scheduling follow-up appointments Refer patients to in-pt. or out-pt. substance abuse treatment programs, partial hospitalization programs, counseling, PCP, med management, CBRS, case management, SUD treatment, PCS services, and home health, etc. Review prospective patient charts, provide opinion to doctor about appropriateness of admission Treatment team meeting with each psychiatrist bi-weekly Facilitate social services group on scheduled days Meet with and call the DE's to discuss patient progress and mental hold status Follow up on the mental hold process to ensure filing of paperwork is timed appropriately and the DE's are meeting their deadlines Fill out and filing physician's holds Write personalized behavior modification plans for patient's as needed; letters for patients; letters for guardianship referrals Assist patients to fill out applications (Medicaid, Social Security, Indigent Funding, Prescription assistance) Fill out PASRR and sending off all chart information for approval for level 2 screening Benefits offered: Medical Benefits Preferred Provider Option Health Savings Account Dental Benefits Vision Benefits Flexible Spending Account Pet Insurance Group Life Insurance, AD&D
