Full Time Job Summary:
We are seeking a compassionate and dedicated Social Service Director to join our team to provide services to our short term and long term guests. As a Social Worker, you will be responsible for assessing the psychosocial needs of our guests and developing care plans that promote their overall well-being. You will work closely with our interdisciplinary team to ensure that our guests receive the highest quality of care.
Essential Functions:
The social worker will work to appropriately transition the resident and family through the care process Implement healthcare directives and ability to have discussions and knowledge of end of life care and hospice/palliative care. Understand fundamentals of discharge planning and able to work collaboratively with healthcare team which includes nursing and physician Conduct psychosocial and cognitive assessments of guests to identify their needs and preferences Develop care plans that address the social, emotional, and psychological needs of guests Collaborate with the interdisciplinary team to ensure that care plans are implemented effectively Provide counseling and support to guests and their families Advocate for guests and ensure that their rights are protected Maintain accurate and up-to-date guest records Participate in team meetings and care conferences Provide education and training to staff and families on social work issues
Qualifications/Experience:
Minimum of 2-3 years' experience in long term care and short term rehab Must hold a Bachelors degree Must demonstrate strong conflict resolution Excellent communication and interpersonal skills Knowledge of federal and state regulations related to long term care Strong organizational and time management skills Compassionate and empathetic demeanor Knowledge of Point Click Care is preferred If you are passionate about making a difference in the lives of others and meet the requirements listed above, we encourage you to apply for this rewarding position.
