Prairie Village Healthcare is looking for a Director of Social Service. Must be familiar with or possess the ability to learn the M.D.S. and state/federal guidelines for Skilled Nursing Facilities.

Applicants must posses skills to:

Conduct intake interview and assessments Assist residents in completion of Advanced Directives and guardianship as needed Participate in the development and review of care plans Facilitate supportive treatment intervention to maintain or enhance resident's capacity for independent functioning Assist as a liaison with outside agencies as needed to facilitate communication and services Assist in coordination of discharge planning services Provide support for behavioral management Accept and follow up on grievance issues reported Maintain close relationship with residents and their family members Maintain concise comprehensive case records by following established agency policies and procedures All other duties as assigned
