Social Services Director Responsibilities:

Maintain a community resource list complete initial assessment Implement care plans, assist with discharge planning Document patient progress Coordinate family meetings Ensure regulatory compliance Complete MDS sections as assigned, ensure all areas are addressed in the care plan as necessary.

Requirements:
