Job Description

Full Time Role Salary:

65,000 to 70,000 (Commensurate with experience) 5 days per week schedule On-call manager rotation

POSITION PURPOSE

To support the activities of the Social Services department while also assisting with the Admissions processes on an as needed basis. Social Services Manager Attends PAN meetings to engage actively in discharge planning. Serve as a liaison with other departments for discharge planning. Track status and progress of appeals. Attend and co-lead care plan meetings. Manage and coordinate goods and services with off-site companies for durable medical equipment, home health visits, and other services required for discharge. Assist in the oversight of the LTC unit. Mentor, train, and provide supervision to other assistants. Serve as acting Director in the absence of the Director.

Requirements:

Bachelor's degree in social work, Healthcare Administration, or related field LSW preferred 3 years' experience providing resident or social service support. 1 year supervisory experience
