Job Description
Social Services Director Skilled Nursing Facility looking for a Director of Social Service who will be responsible for planning, organizing, developing and directing the operation of the Social Service Department in accordance with federal & state guidelines and regulations. Duties include but are not limited to:
- Admission and Discharge Planning
- Resident Care Plans and Assessments
- Process Medicaid Application & Follow Up With COMRU
- Ensure that Social Service Componets of MDS are Completed
- Investigate & Review Residents Complaints & Grievances
- Educate & Inform Residents of Their Rights
- Provide Residents & Family Members With Resources In The Community To Meet Their Needs
- Develop & Implement Polices & Procedures For Residents Social & Emotional Needs The idea candidate will have knowledge of rules, regulations and guidelines governing a nursing home.
