Job Description

DIRECTOR OF SOCIAL SERVICES
Bedford Hills Center Bedford Hills Center is seeking a compassionate, organized, and experienced Director of Social Services to lead our Social Services Department. This leadership role is responsible for ensuring residents and families receive the support, advocacy, and resources necessary to enhance quality of life and promote successful care outcomes. The Director of Social Services works closely with residents, families, interdisciplinary team members, and community resources to address psychosocial needs and facilitate resident-centered care.
Salary:
$66,000 - $80,000 annually, based on experience
Responsibilities Include:
Directing and overseeing all social services functions within the facility Completing psychosocial assessments and developing individualized care plans Assisting residents and families with adjustment, discharge planning, and community resource coordination Participating in interdisciplinary care plan meetings Advocating for resident rights and promoting resident satisfaction Providing counseling and support to residents and families as appropriate Ensuring compliance with federal, state, and facility regulations Supervising social services staff and departmental operations as applicable
Qualifications:
Bachelor's Degree in Social Work, Human Services required Minimum of 1 year of supervised experience in a healthcare setting required Long-term care or skilled nursing experience preferred Strong communication, interpersonal, and organizational skills Ability to work collaboratively with residents, families, and interdisciplinary team members Knowledge of discharge planning, community resources, and regulatory requirements preferred
