Job Description

Rose Mountain Care Center , centrally located in New Brunswick, NJ at is looking for a Full Time license Bilingual Chinese/English Social Services Director with experience to join our dedicated team of healthcare professionals.
Requirements :
Bachelor's degree required. Master's degree strongly preferred. LSW or CSW Licensure Required
EXPERIENCE WITH MDS 3.0 / PHQ-9
/ BIMs Long-term care / sub-acute experience Responsibilities include but are not limited to: Develop preliminary/comprehensive assessments for each resident. Encourage the resident/family to participate in the development and review of resident's plan of care. Discharge planning Develop a written plan of care for each resident that identifies the social problems as well as the needs of the resident and set goals to be accomplished for each problem/need identified. Ensure that the social services components of the MDS assessment are completed and signed in an efficient manner. Review nurse's notes to determine if the care plan in place is being followed. Ensure quality customer service. Timely completion of resident assessments The social worker will work with residents by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Services will be provided in accordance with the National Association of Social Workers (NASW) Code of Ethics along with strict adherence to government regulations. The social worker is responsible for fostering a climate, policies and routines that enable residents to maximize their individuality, independence and dignity. This climate shall provide residents with the highest practical level of physical, mental and psychosocial well-being and quality of life. #IND123
