This is a leadership role responsible for overseeing social services functions, discharge planning, and psychosocial support within our facility.

Responsibilities:

Lead and manage the Social Services department Ensure compliance with state, federal, and facility regulations Oversee resident admissions, discharges, and care transitions Support residents and families with psychosocial needs and adjustment Coordinate with nursing, therapy, and administrative teams Maintain accurate documentation and required assessments Advocate for resident rights and quality of life initiatives

Qualifications:
