Job Description
The Village at Morrisons Cove, a continuing care retirement community, is currently seeking to hire a Director of Social Services for our skilled nursing facility. The Village at Morrisons Cove was established in 1903 and has served older adults in our community ever since. The Village is home to approximately 75 residents, with the capacity for 114. We are looking for Director of Social Services who will provide assistance to our residents and their families. Some of the primary job functions of this role include, but are not limited to: Timely completion of the social service portions of the MDS Completing initial assessments on new admissions Completing quarterly and annual assessments Participating in and documenting resident care plans in conjunction with the interdisciplinary team Identifying, monitoring and treating residents with behaviors Facilitating room moves including communication with residents, families and supporting documentation Working with outside agencies to procure necessary services for the residents Completing ADRs Issuing Notice of Medicare Non-Coverage and assisting with appeals if warranted Working directly with hospice agencies to coordinate services Coordinating discharge planning for residents Acts as a back-up to the admission process Completing required social service documentation on residents Qualified candidates must hold at minimum a bachelor's level degree in social work or related field (example: health and human services, psychology, sociology etc). Experience working with geriatrics or older adults is preferred. This position is full time-exempt, Monday-Friday, and is eligible for full time benefits.
