Full Job Description Responsibilities:
  • Provide social work services to geriatric residents and their families
  • Conduct psychosocial assessments and develop individualized care plans
  • Collaborate with interdisciplinary teams to coordinate patient care
  • Provide counseling and support to patients and families dealing with PTSD or other mental health issues
  • Assist with case management, including referrals to community resources and follow-up care
  • Collect and analyze data for quality improvement initiatives
  • Ensure compliance with HIPAA regulations and maintain confidentiality of patient information
  • Complete medical documentation, including coding using ICD-10
Qualifications:
  • Bachelor's or Master's degree in Social Work or related field
  • Knowledge of psychopharmacology and its impact on patient care
  • Familiarity with HIPAA regulations and guidelines for protecting patient privacy
  • Strong understanding of PTSD care and trauma-informed practices
  • Proficient in case management principles and techniques
  • Experience with data collection, analysis, and reporting
  • Ability to accurately code medical documentation using ICD-10
