Full Job Description Responsibilities:
- Provide social work services to geriatric residents and their families
- Conduct psychosocial assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to coordinate patient care
- Provide counseling and support to patients and families dealing with PTSD or other mental health issues
- Assist with case management, including referrals to community resources and follow-up care
- Collect and analyze data for quality improvement initiatives
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information
- Complete medical documentation, including coding using ICD-10
Qualifications:
- Bachelor's or Master's degree in Social Work or related field
- Knowledge of psychopharmacology and its impact on patient care
- Familiarity with HIPAA regulations and guidelines for protecting patient privacy
- Strong understanding of PTSD care and trauma-informed practices
- Proficient in case management principles and techniques
- Experience with data collection, analysis, and reporting
- Ability to accurately code medical documentation using ICD-10
