Position Summary We are seeking an experienced Social Worker to join our leadership team as a Skilled Nursing Social Services Director. In this role you will lead the social services functions, ensuring exceptional psychosocial care for residents while collaborating closely with our interdisciplinary team. With the support of a Social Services Assistant, you will play a vital role in coordinating care, supporting families, and driving positive outcomes. Key Responsibilities Plan, coordinate, and lead IDT care plan meetings Investigate, follow up, and resolve grievances Oversee discharge planning and transitions of care Supervise and support the Social Services Assistant Build and maintain strong relationships with residents and their families, clinical and non-clinical team members, cluster partners and referral sources, marketers and admissions teams Advocate for residents' psychosocial well-being and rights Ensure compliance with state and federal regulations
