Job Description
The Pines Rehabilitation & Health Center is a 60 Bed SNF/NF facility that provides Quality and Compassionate Resident Centered Care to the residents we serve. We're located in Beautiful Lyndonville, VT. We are currently accepting applications for a Full Time Social Services Director . As the Social Services Director, your primary role includes: Collaborating with the interdisciplinary team, nursing and administration to promote and protect patient rights, dignity, independence and psychological well-being Maintaining good working relationships with center employees, residents and families Assuring a comprehensive Psychosocial Assessment is completed for each patient that identifies social, emotional, psychological needs and strengths. Educating patients and families regarding their rights and responsibilities, health care decision making/advance directives, effective problem solving, and available community resources Providing therapeutic interventions to assist patients to cope with their transition and adjustment to a long-term care facility and to address behavior or mood problems Working with facility personnel, families and residents in completing any necessary insurance enrollments, prior authorizations and coordination of Medicaid applications. Candidates must have the following qualifications: Bachelors Degree in the field of Social Services/psychology preferred A desire to be an active voice and advocate for the residents and neighbors of the community we serve Basic computer and software skills required We offer a competitive salary and a comprehensive benefits package including Paid Time Off, Health/Dental/Life/Disability Insurance, and 401(k).
