Job Description

Overview We are seeking a compassionate and highly organized Peer Support Navigator for our Permanent Supportive Housing program. The primary role is working with individuals who are homeless and in need of housing and support services. The Navigator serves as the bridge for the individual in coordinating and monitoring housing-based services. based services and facilitates delivery of these services by supporting the individual The Navigator works in conjunction as part of a multi-disciplinary team made up of other Community Agencies and stakeholders. Duties Provide individualized services to clients. Transport clients to interviews, seraching for housing, and other community needs Conduct annual biopsychosocial assessments Develop and monitor personalized recovery. Coordinate care with other stakeholders and multi-disciplinary teams. Assist clients with referrals housing, transportation, and community resources Facilitate and coordinate staffings among the client's support teams. Maintain accurate, timely clinical documentation in electronic health records Advocate for clients within medical and recovery systems.
Required:
Minimum 1 year experience in PSH or behavioral health services Strong documentation and organizational skills Ability to manage caseload Reliable personal vehicle for in-person transportation services
Fingerprint Clearance Card Preferred:
Peer Support certification Experience working with high-acuity, medically complex clients, or unhoused individuals Skills & Competencies Strong interpersonal and motivational interviewing skills Trauma-informed and culturally responsive approach Excellent organization and time management Professional boundaries and ethical judgment Ability to collaborate across medical and behavioral health systems Strong written and verbal communication Empathy, patience, and resilience
Job Types:
Full-time, Contract Pay:
$20.00 - $22.00 per hour
