OVERVIEW
Provides case management for housing programs for low-income individuals and families with children. Conducts intake and assessment of clients, determines eligibility for available affordable housing programs, and monitors client progress. Provides advocacy and follow-up assistance. Maintains necessary case records and client file documentation. Provides information and referrals. Ensures program meets contract requirements. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned.
ESSENTIAL FUNCTIONS
Processes self-referrals and referrals from community agencies by providing initial screenings, intakes, interviews, and eligibility assessment of participants Develops individual service plans in partnership with clients; monitors progress and implements follow-up activities Provides referrals, assists the client in arranging appointments with community resources or private group counseling sessions and monitors client progress Maintains individual service files for clients that include plans and strategies for achievement of housing goals and provides follow up contacts to assess progress May drive agency or personal vehicle on company business
MINIMUM REQUIREMENTS
Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Executive Director of Human Resources. Associate Degree plus two (2) years of Behavioral Health work experience (AHCCCS requirement for BHT); or High School Diploma or GED plus four (4) years of Behavioral Health work experience (AHCCCS requirement for BHT) 1 year of experience working with housing programs, populations at risk of homelessness, and/or low-income individuals and families Excellent data entry skills (speed and accuracy) Proven skills utilizing Microsoft Office Suite Excellent organizational skills Proven strong communication skills - oral and written communications are clear and concise using appropriate style, grammar, and tone
REGULATORY
Must be at least 18 years of age Valid driver license, proof of insurance, and 39-month motor vehicle report Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report (employer paid) Pass pre-employment drug screen (incudes marijuana, regardless of recreational use laws) (employer paid)
DESIRED QUALIFICATIONS
