POSITION OVERVIEW
As a Hospice Liaison, you will serve as a trusted ambassador for Tranquil Oaks Hospice Care within the medical community. Working closely with our Admissions Team, you will connect patients and families to compassionate hospice services while building strong partnerships with referral sources.
Key Responsibilities:
Evaluate patients for hospice eligibility as ordered by physicians Guide patients and families through informed decisions regarding hospice services Obtain admission agreements for qualified patients Support patient intake and ensure completion of required documentation Plan and participate in educational activities with referral sources Build and maintain strong professional relationships within the healthcare community Communicate confidently and effectively in both one-on-one and group settings Perform additional responsibilities that support agency growth and patient care We are seeking a motivated professional who combines clinical understanding with strong relationship-building skills.
QUALIFICATIONS
Minimum of 2 years of hospice or healthcare sales/marketing experience preferred Current CPR/First Aid certification Licensed RN, LVN, or MSW preferred Excellent verbal, written, and interpersonal communication skills Professional phone etiquette and strong telephone communication skills Strong organizational skills with the ability to multitask effectively Team-oriented mindset with a collaborative spirit Valid driver's license and reliable, insured transportation
