Job Description

Assess new members at admission for service needs & preferences Develop service plan & change as needed periodically Assist member in transition to ACV community & monitor regularly for effectiveness of service plan Maintain accurate records Maintain contact with family members and other significant individual(s) on behalf of the member Minimum of two years of relevant experience strongly preferred Bachelor degree from fully accredited university in relevant senior social services field required
  • Must work well with senior adults
  • Valid Florida driver's license required
