The Social Services Specialist responsibilities include: Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs Possess a clear understanding of discharge and transition planning and linkage with community resources Demonstrates skills in planning, organizing, and managing multiple functions and complex processes Position details:

Full time, 40 hours per week
Schedule:
Monday•Friday 8:00 AM•4:30 PM Weekend requirement:
Occasional On Call:
No Certifications and Licensures:
Preferred ACM (Case Management) or Preferred CCM (Case Manager)
Education:
Required Masters in Social Work, Sociology, or Related Field Strongly Preferred Masters Degree in
Social Work Experience:
Required 6 months Internship or Required 1 year Social Work in an acute care or hospital setting
