Job Description

Social Services Specialist (223623-14) We are seeking a compassionate social work professional dedicated to supporting patients and families through care coordination, advocacy, resource navigation, and community-based services. Hours Full-time, Monday-Friday, 7:45am-4:45pm Location Honolulu, Oahu Pay $28 to $31 an hour Responsibilities Connect patients and families with community resources, financial assistance programs, and support services. Arrange transportation, medical appointments, facility transfers, and discharge plans. Conduct assessments of discharge evaluations to ensure appropriate care placement. Coordinate referrals and collaborate with healthcare providers, community agencies, and support organizations. Provide guidance and support to patients and families during medical crises, end-of-life situations, and bereavement planning. Assist with legal, financial, and administrative matters, including advance directives, benefits, and required documentation. Develop care plans and participate in interdisciplinary case conferences. Maintain accurate patient records, case notes, and care documentation. Communicate regularly with families, healthcare teams, and community partners to support patient care. Facilitate caregiver support groups, community outreach efforts, and educational activities. Participate in team meetings, training programs, and other duties that support patient services. Qualifications At least one year of professional social work experience. Healthcare or care management experience preferred. Ability to work with diverse populations and complex family situations. Knowledge of social service programs, community resources, and benefits systems. Experience maintaining documentation and managing multiple cases. Ability to make sound independent decisions while working collaboratively with a team. Valid driver's license required. Bachelor's Degree in Social Work required. Master's Degree in Social Work preferred.
