Housing Specialist:
This is a part-time position. Day shift.
Primary duties:
Help clients gain access to needed medical, social educational and other services. Provide direct assistance in gaining access to services, coordination of care, oversight of the entire case and linkage to appropriate services.
Areas of service:
Assessment of the eligible individual to determine housing needs Responsible for accurate and timely documentation related to grant deliverables Referral and related activities to advocate and assist the individual obtain needed services Monitoring and follow up of the housing plan Linkage and networking with court, health care providers, employers and other community agencies Provide integration and assistance to clients and families to help sustain within the community/least restrictive environment Accountable for the flow of information within a multidisciplinary team setting Special Populations/Skills/Employment duties, as assigned.
Requirements:
Associate degree required with two years of related and relevant experience required; BA/BS preferred. Experience with targeted population highly preferred. Valid driver's license Proof of insurance
