Job Description

TCM/WAF Targeted Case Manager/Wraparound Facilitator Full Time General Responsibilities:
The Wraparound Facilitator/TCM Specialist works in Children's Community Based Services of Compass BH. The primary function of the Wraparound Facilitator/TCM Specialist is to provide medically necessary services as set forth in the consumer's treatment plan/ plan of care. The WAF/TCM will coordinate, monitor and complete all waiver paperwork for CBS waiver consumers. This position will be responsible for helping individuals access needed medical, social, educational, and other services. The Wraparound/ TCM Specialist will be responsible for developing wraparound teams and plans of care, monitoring treatment plans/plans of care and progress notes to assure the plan is working and make changes as needed.
MINIMUM QUALIFICATIONS
A BA/BS degree with an emphasis in mental health/human services or be equivalently qualified by work experience or a combination of work experience in the human services field and education with one year experience substituting for one year of education. Wraparound provider shall possess demonstrated interpersonal skills, ability to work with SED children/adolescents/families and the ability to respond effectively in a wide variety of human service situations. Must be at least 21 years of age and have a current and valid driver's license. Employee must successfully complete Wraparound Certification as stipulated by the State of Kansas within six months of employment.
CERTIFICATES, LICENSES, REGISTRATIONS
Employee is required to complete appropriate on-line certifications through Training Teams prior to billing of services. Must attend Interactive Community Events (ICE) Training within six months of hire. Required to complete CPR/First Aide/CPI within compliance timeframes.

While performing the duties of this position, the employee is exposed to a typical office environment and interaction with many different persons. In addition, the employee travels by automobile and is exposed to changing weather conditions. Must have a valid Driver's License and be insurable under the Center's policy in order to operate a Center vehicle. The employee will interact with many different persons in varying stages of recovery from their mental illness.
