Job Description

Assertive Community Treatment Team Housing Specialist- Full‑
Time Location:
Newark, MD Salary Range:
$40,000-$55,000 annually
Schedule:
4‑day, 36-hour work week — Monday through Thursday Position Summary This position will be responsible for planning and implement housing services for clients who have co-occurring mental illness and substance abuse disorders. This position is fully dedicated to CCBHC Expansion Grant. Key Responsibilities Maintain strict confidentiality in all client interactions and documentation, ensuring full compliance with
HIPAA, CARF
standards, and Maryland state regulations. Participate in agency-required trainings and professional development activities as directed by the supervisor. Attend weekly agency housing meetings and relevant community housing forums; build and maintain collaborative relationships with local shelters and housing providers. Contribute to the development and implementation of person-centered treatment plans, including ongoing monitoring and accurate documentation of client progress. Provide case management and supportive counseling services, assisting clients with symptom management and housing stability. Adhere to ACT evidence-based practice standards by delivering high-quality, recovery-oriented services and maintaining a minimum of 15 hours of direct client contact per week. Coordinate care with clients' natural supports and collaborate with community service providers to promote treatment engagement and stabilization. Perform additional duties as assigned by the supervisor to support team and program operations. Qualifications High School Diploma or equivalent Current CPR and First Aid certification. Valid full driver's license with a safe driving record. Excellent verbal and written communication skills. Experience working with vulnerable populations. Why Join HealthPort?
