Community Support Team Qualified Professional Continuum Care Services, Inc. - Winston - 3.9 Winston-Salem, NC Job Details Part-time | Full-time $18 an hour 19 hours ago Qualifications Care planning in social services Driver's License Bachelor's degree Full Job Description Job Description Job Description Job Summary Continuum Care Services Inc. is seeking a empathetic, compassionate, positive and personable individual to join our Community Support Team (CST) as the Qualified Professional (QP). The (QP) is to provide Community Support Team services that will include direct and indirect interventions. The development of the Person-Centered Plan is the responsibility of the QP on the team. CST is intended for individuals with mental illness, substance abuse, disorders, or both who have complex and extensive treatment needs. As a service provider, the Qualified Professional must assist the consumer to develop critical daily living and coping skills, gain access to necessary services, and reduce psychiatric and addiction symptoms. This includes, but is not limited to: assessing client needs, arranging services, implementing comprehensive service plans, working in conjunction with Community Support Team Leader (LP), paraprofessional and associate professional staff, advocating on behalf of the client, completing required documentation, and other duties as assigned.:

Bachelor's degree (in Human Services field) 2+ years full-time experience with population served Bachelor's degree (Not Human Services field) 4+ years full-time experience with population served Master's Degree or Higher 1+ years full-time experience with the population served..
Experience:
Develops the initial Person Centered Plan and its ongoing revisions, and ensures the implementation of the Person Centered Plan (PCP). Assists the Team Leader with behavioral and substance abuse treatment interventions Assists with the development of relapse prevention and disease management strategies linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric services Assists with crisis interventions Consults with identified medical (for example, primary care and psychiatric) and non-medical providers, engages community and natural supports, and includes their input in the person-centered planning Occupational Certification/Licenses North Carolina Driver's License Why Continuum Care Services Our company is based on the belief that our customers' needs are of the utmost importance.
