Posting Summary Rutgers, The State University of New Jersey, is seeking a Peer Support Specialist III (Intensive Community Work (iCW)) in the Department of Medicine within the New Jersey Medical School (NJMS). The primary purpose of the Peer Support Specialist III (intensive Community Work) is to connect and engage clients either lost o follow-up or who have never accessed care. Serves a a a role model to demonstrate recovery and wellness principles and provide non-clinical support to clients to pursue and maintain their health and wellness and reach their fullest potential. Serves as a community liaison and work a par of the medical case management team to address barriers to connection and retention in care. Supports the care team through an integrated approach to care management and community outreach. Provides reliable information, appropriate referrals, and emotional support to those clients who have agreed to participate in the program. Helps clients to access services and resources and address issues of adherence through individual and group sessions. Among the key duties of this position are the following: Responsible for introducing community health worker services to all newly diagnosed and new to clinic patients. Assist the medical case management team with caseloads of newly diagnosed and lost to follow up patients. Assist the medical case management team with collection of necessary documents and submission of applications for housing, emergency financial assistance and medication assistance. Manages an assigned caseload. Provides intensive adherence counseling to patients that are not virally suppressed >6 months.

40.00 Work Arrangement Consistent with the current application of Rutgers Policy 60.3.22 or the applicable provisions of relevant collective negotiations agreements, this position requires a fully on-site work arrangement and may be eligible for a compressed workweek or a flex workday schedule. Flexible work arrangements are not permanent, are subject to change or discontinuation, and contingent on the employee receiving approval in the FlexWork@RU Application System.

PERS Minimum Education and Experience High School Diploma or equivalent required. Must have three (3) years of training and/or experience involving knowledge of the Newark area and community resources, firsthand understanding of issues related to living with HIV or AIDS, and/or active participation in community activities and support groups. Demonstrated ability to work with a diverse group of people regardless of their creed, race, and sexuality required. Preferred Qualifications Sensitivity to a substance abuse environment preferred. PHYSICAL DEMANDS
Position requires walking 2/3 of the time, requires sitting 2/3 of the time, use of hands to fingers, handle or feel over 2/3 of the time, reach with hands and arms 1/3 of the time, 2/3 of the time talking or hearing.
WORK ENVIRONMENT
Moderate noise (examples: business office with computers and printers, light to moderate traffic). Exposure to blood-borne pathogens that requires use of the personal protective equipment.
