Enrollment Specialist - Home & Community Services Position Summary We are seeking a dedicated and compassionate Enrollment Case Manager (ECM) to support individuals in accessing Home and Community-Based Services (HCBS) programs. This role involves working closely with applicants, primarily in their homes, to guide them through the eligibility and enrollment process.

Pay Rate:

$20.50/hour (fixed)

Benefits Mileage Reimbursement:

$0.70 per mile

Weekly Pay:

Includes salary and gas reimbursement Job Overview Work primarily in applicants' residences Travel within assigned counties and nearby areas as needed Facilitate the enrollment process for HCBS programs Engage directly with applicants to assist with eligibility and onboarding

Note:

Must have a valid driver's license and personal vehicle Public transportation is not permitted for this role Key Responsibilities Travel within assigned and neighboring counties to meet applicants Conduct intake visits for HCBS applicants Guide applicants through the eligibility determination process Educate individuals on HCBS programs, including rights and responsibilities Assist with application submission, including COMPASS data entry Help applicants select appropriate: Managed Care Organizations (MCOs) Life providers Support coordinators Maintain relationships with: Healthcare providers County Assistance Offices Nursing homes and rehabilitation centers Collaborate with community organizations and stakeholders Monitor application progress and resolve stalled cases Participate in scheduled meetings and provide status updates Perform administrative and reporting tasks Occasionally lift and carry items up to 25 lbs Execute additional duties as assigned Qualifications (Preferred) Strong communication and interpersonal skills Ability to work independently and manage travel schedules Experience in healthcare, case management, or social services preferred Basic computer skills for data entry and reporting Why Join Us?
