Key Responsibilities The Division of Services for People with Disabilities (DSPD), within the Department of Health & Human Services, is looking for a dedicated professional to fill our Caseworker Specialist II position. As a Caseworker Specialist II on the Intake and Waiting List team you will provide support to individuals with Developmental Disabilities or Acquired Brain Injuries going through the application process or on the waiting list for DSPD services. Support includes responding to calls and emails from clients, advocates, or families; reviewing documentation to determine if individuals meet program eligibility; updating annual assessments; linking with community resources to help meet needs while on the waiting list; and completing referrals for emergency services when appropriate. Other essential job duties include attending community events targeted toward individuals with disabilities, participating in multi-agency committees, and presentations to community partners. Intake and Waiting List Caseworkers have large caseloads and must be able to prioritize tasks, respond to calls and emails within expected time frames, and maintain accurate documentation within their caseload files. The individuals and families we support may be dealing with difficult circumstances that result in heightened emotions at times and Caseworkers may have to employ techniques to de-escalate discussions. Examples of Duties Review and evaluate psychological evaluations, developmental assessments, and/or medical documentation to determine program eligibility. Assist with requesting documentation to help support an individual's application. Complete initial and annual Needs Assessment Questionnaires and Waiting List Surveys. Complete Comprehensive Brain Injury Assessments. Assess situations and provide resource information to prevent or resolve crisis situations. Attend school transition fairs, community resource events. Documents interaction with clients and/or advocates in a timely and professional manner. Represent DSPD on multi-agency committees to answer questions about our services, intake/eligibility, and waiting list processes. Assist with covering the intake phone line 2-3 times per month. Support and sustain DSPD goals, plans and objectives. Qualifications Minimum Qualifications Has experience supporting individuals with disabilities to lead self-determined lives. Understands Division services. Has strong interpersonal skills. Has the ability to work well independently. Is an excellent listener who is also proactive. Has the ability to work well with a wide range of people and agencies with varying interests. Preferred Qualifications Preference may be given to individuals with a Bachelor's degree in a related field. Has knowledge of Home and Community-Based Services (HCBS), Medicaid and Medicaid Waivers. Generally, the best qualified individuals for this job will have previous experience with the community or assigned client base. Human Services (DHS) generally requires licensure as a Social Service Worker if employee works on a regional team that provides direct social welfare services. Other divisions within DHS may also require certification(s) depending on the area of assignment. If the assignment includes working with deaf or hard of hearing individuals, the ability to communicate fluently in sign language may be preferred. Risks found in the typical office setting, which is adequately lit, heated and ventilated, e.g., safe use of office equipment, avoiding trips and falls, observing fire regulations, etc. Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
