About Us:
Care Partners is a leading healthcare organization dedicated to improving the lives of vulnerable individuals through innovative care management, home-based services, and community partnerships. As a provider of Enhanced Care Management (ECM) services under California's CalAIM initiative, we work alongside health plans, healthcare providers, and community organizations to support high-risk Medi-Cal members with complex medical, behavioral, and social needs.
Our mission:
To provide care for those in need through Love, Compassion, and Empathy. This role combines healthcare sales, community engagement, relationship management, and field-based member assessments. The successful candidate will develop referral partnerships, maintain enterprise-level account coverage, conduct in-person outreach, perform member assessments, and serve as a trusted resource regarding CalAIM, Enhanced Care Management, and Medi-Cal services.
Community Liaison Requirements:
EDUCATION AND EXPERIENCE
Minimum 2 years of experience in healthcare sales, business development, physician relations, community outreach, account management, home health, hospice, or related healthcare field. Proven ability to build and maintain professional relationships. Strong communication, presentation, networking, and interpersonal skills. Reliable transportation and ability to travel extensively throughout North Orange County and Los Angeles County. Preferred Qualifications Experience working with Medi-Cal populations. Knowledge of CalAIM, ECM, Community Supports, managed care, or population health programs. Existing relationships with hospitals, SNFs, FQHCs, physician groups, health plans, community-based organizations, or social service agencies.
Essential Functions:
Develop and maintain referral relationships with healthcare providers, health plans, and community organizations. Conduct field outreach, presentations, and networking activities to generate referrals and increase community awareness. Educate referral partners and members on ECM, CalAIM, and Care Partners services. Manage referrals, complete member enrollments, and coordinate service initiation. Represent Care Partners at community events, resource fairs, and industry meetings. Utilize HubSpot CRM to track outreach activities, referrals, and account growth. Drive territory growth through strategic relationship building and community engagement. Additional Information All your information will be kept confidential according to EEO guidelines.
