Job Description
Gateway Unlimited Living LLC is hiring Targeted Case Managers to provide Housing Case management services with Moving Home Minnesota and Relocation Coordination for individuals who reside in eligible institutions. Assisting with moving from the institution, to a community-based setting. We are hiring for the following locations: Twin Cities, Wabasso, Redwood Falls, Olivia and Franklin. Must be willing to travel to all locations. This position is a form of Targeted Case Management that provides coordination of services to help a person who resides in an eligible institution gain access to medical, social, educational, financial, housing, and other services and supports that are necessary to move from such institutions to the community.
Services May Include:
Targeted Case Management duties Relocation Service Coordination & Case Management dutiesMoving Home Minnesota Required Job Responsibilities:
- Outreach to case managers / care team members to ensure that the requested services are appropriate
- Expand and connect with care team to receive all appropriate release of information
- Support the person served and their chosen caregivers in developing a plan for long term stability
- Provide Person-Centered Planning offering a customized plan including housing, expungement, transportation, and community resources.
- Having knowledge of geographical areas and housing resources and tools to support a person's plan for stability after discharge
- Coordination and collaboration with other Targeted Case Management services as well as managed care organizations
- Knowledge of local community resources and hot to use those resources for the benefit of the person
- Developing and reviewing a relocation plan with a person
- Making referrals to needed services and supports
- Communicating with the person to carry out the relocation plan in the 180-day period before discharge
- Coordinating overall service delivery to ensure quality of services and maintaining compliance as needed.
- Completing documentation necessary to develop and carry out the relocation plan Travel to see clients face to face.

- The ability to ensure the service agreement is correct and meets all DHS requirements

Physical and Mental Demands:
- Good physical and mental health.
- Physical ability to stand, walk, use hands and fingers, reach, stoop, kneel, and crouch
- Mental fortitude and stability to handle stress.
- Physical and mental ability to drive a vehicle
- Ability to lift a minimum of 25 pounds
- Ability to communicate clearly with staff and consumers verbally or utilizing other effective means
Required Qualifications & Education:
- Current valid driver's license
- Reliable vehicle and ability to transport clients (company will reimburse authorized mileage)
- Proof of proper vehicle insurance
- Bachelor's degree in social work, nursing, psychology, sociology, or a closely related field
- MUST pass Background Study and DHS Fingerprinting
- Experience in areas of employment directly related to the performance of this job as included in the above "Job Duties and Responsibilities"
