Job Description

The Recovery Support Specialist uses the SMART Recovery curriculum to engage individuals with addictive behaviors, including both substance and activity/process addictions, to build skills to manage addictive behavior and facilitate recovery, resiliency, and wellness. This is a hybrid position with some time working in a Social Care Navigator capacity. Those duties will involve working with the Medicaid 1115 program to help navigate eligible customers to existing or enhanced services to meet their Health-Related Social Needs.

Owego, NY Generous benefits include: 12 days of vacation Sick and personal leave 12 paid holidays 403b retirement savings 403b agency contribution Health, dental, vision insurance with agency contribution Life insurance; Long-term disability insurance Responsibilities of the Recovery Support Specialist position include: Train to become a SMART Recovery facilitator and maintain certification Work 1:1 and in small groups to lead effective discussions on: Building and maintaining motivation Coping with urges and cravings Managing thoughts, feelings, and behaviors Living a balanced life Develop and provide tailored community education programs on healthy behaviors that promote overall wellness. Coordinate education and outreach services with other community programs to promote comprehensive services and to prevent the duplication of services Assure that standards for delivery of educational programs are met. Work with agency staff to identify community supports and help clients/consumers understand how to utilize these resources in the recovery process. Develop and implement Individualized Recovery Plans. Model coping techniques, self-help strategies and reinforce the potential for recovery to clients/consumers. Incorporate integrated service delivery model while promoting healthy self-reliance Maintain awareness of community resources and TOI programs/services; make appropriate referrals as needed with documented follow-up Regularly assess, monitor, and report customer progress and stability toward individual goal and outcome achievement Administration of program assessments and re-assessments Operate from a strength-based, customer-driven model. May accompany customers to service-related appointments and/or community events Maintain timely and accurate documentation of all customer service activities, and ensure that customer records are complete and accurate Enter customers into required data systems Participate in the larger team of the agency through committee structures, planning, policy development and volunteerism. Collaborates with the Community Relations & Marketing Manager to identify, conceptualize, design, and execute campaigns to boost engagement, educate, and inform TOI's audience When assigned to assist with the Medicaid 1115 Waiver program: Assess Medicaid members using approved NYS Health Related Social Needs Screening Questionnaire Navigate eligible customers to existing or enhanced services to meet their Health-Related Social Needs Send and respond to service referrals via the Unite Us platform Document customer contacts and service delivery within the Unite Us platform Case management will be a key part of the role, working directly with individuals/families.This includes:
Talking with community member(s) to help them identify needs Creating personalized goals and planning action steps in partnership with families/individuals Collaborating with other entities such as health care providers, landlords, human service organizations, and other community resources Documenting client interactions, progress, and monitoring following up Coordinate/authorize rent and/or utility payments, as needed Support customers with Pre-tenancy and Tenancy Sustaining Services (including housing searches) Coordinate private transportation for eligible customers to locations relating toHealth-Related Social Needs Required Knowledge, Skills, and Abilities:
Preference will be given to those with a lived experience in the area of addiction recovery Strong interpersonal, communication, engagement, and customer relations skills Demonstrates respect of privacy and confidentiality of client interviews, information, and files Goal and team oriented Ability to develop and maintain positive relationships with people from various backgrounds Must be able to provide consistent, quality customer service at all times Must be calm, flexible, organized, and able to work in an office setting Must have computer skills to perform efficiently, i.e. MS Office; data entry Flexibility in work schedule Other facts On-site attendance is a requirement of this position Must have a clean, current driver license that is acceptable to Agency standards and ability to travel throughout the county Standard hours are 8:30-4:30 Monday - Friday; flexibility in schedule is allowed to the extent that the customer needs are metMinimum Education and Experience:
A graduate from a regionally accredited registered four-year college or university with a bachelor's degree in social work or human services related field AND one year of full-time paid experience in case management; OR A graduate from a regionally accredited or registered two year college with an associate degree AND three years of experience in community health services, or human services or a related field: OR An equivalent combination of training and experience as indicated above.
