Job Description

Job Overview:
Credible Health Solutions (CHS) is seeking a dedicated and motivated QMHP-CS to provide Mental Health Targeted Case Management (MHTCM) services to children and adolescents in the Galveston County area. This role is community-based and field-driven , meaning services are provided in the client's home, school, or community setting . The QMHP-CS will work directly with clients and families to improve functioning, build life skills, and connect them to essential resources. This is a rewarding opportunity to make a direct impact in the community while growing professionally in behavioral health. Responsibilities; Skills Training (H2014): Provide skills training using approved Texas 15.3 curriculum models , including:
Youth-Focused Models:
START (Social Skills & Aggression Replacement Training) PAYA (Preparing Adolescents for Young Adulthood)
Seeking Safety Nurturing Parenting Program Barkley's Defiant Child/Defiant Teen Adult-Focused Models:
Illness Management and Recovery (IMR) Assertive Community Treatment (ACT) principles Individual Placement and Supports (IPS - Supported Employment) Permanent Supportive Housing (PSH)
Core Skills Training Functions:
Teach coping skills, emotional regulation, and problem-solving Support independent living skills (adults) Assist with employment readiness and daily functioning Provide caregiver (LAR) support and involvement when applicable Case Management (T1017): Link clients to community resources (SSI/SSDI, housing, employment, medical care) Coordinate care with providers, schools, and community agencies Advocate for client needs across systems
Documentation & Compliance:
Complete BIRP notes within 48 hours of service Ensure all services are tied to the Individualized Service Plan (ISP) Maintain compliance with TMHP, Medicaid, and HIPAA standards
Additional Duties:
Participate in supervision and required trainings Maintain productivity standards Follow all ethical and professional guidelines
Pay:
$20.00 - $25.00 per hour Application Question(s): Bachelor's degree in psychology, Social Work, Counseling, Criminal Justice, or related field
