Job Duties:

This HSS VII position is assigned to the Stiles unit, while providing services for other units, as needed. You will work directly with inmates who have diagnosed medical and/or mental health needs and are preparing for release from prison. It is important that you can advocate for the inmates as you navigate the medical, mental health and correctional systems. You will sometimes need to provide assistance and support to inmates who may be unable to participate or communicate their own needs.



The HSS VII is tasked with identifying any barriers to a successful transition to the community and putting together a continuity of care plan for the inmate upon release. You will review medical and mental health records, speak with unit medical staff, family, and the inmate to determine specific needs. You will coordinate with family, community partners and community resources to link the inmate to services, Social Security benefits and other supports for which they may qualify.



You will serve as the unit's primary point of contact for the Medically Recommended Intensive Supervision Program.



In addition, you will keep records up to date, complete reports and correspondence, and review data to prepare summaries. This role requires you to stay organized and make sound decisions and work independently while still collaborating with others when needed.

You have 4 years of related work experience and a bachelor's degree or 8 years of related work experience.



You have experience working in a medical or mental health environment.

You have experience in case management and medical and/or mental health patient experience.



You are proficient in Microsoft Office 365 applications and can use a computer effectively for documentation, data management and communication.



You can build productive relationships with unit staff, unit medical, providers, and community resources.



You can explain continuity of care and why it is important.



You have knowledge of medical terminology.



You have knowledge of community resources.



You are self-motivated, take initiative and can remain on task independently.



You have 4 years of related work experience and a bachelor's degree.



You have criminal justice experience.
