Hospice Community Liaison Employment Type:
Full-Time Position Summary The Hospice Community Liaison is responsible for driving growth by developing and maintaining strong relationships with physicians, healthcare facilities, community organizations, and other referral sources. This role focuses on educating potential partners about hospice services, promoting compassionate end-of-life care, and ensuring patients and families receive the support they need. Essential Responsibilities
  • Develop and implement strategies to increase hospice referrals and community awareness.
  • Build and maintain relationships with physicians, hospitals, skilled nursing facilities, and community organizations.
  • Conduct educational presentations on hospice services and benefits for patients and families.
  • Collaborate with the clinical team to ensure smooth transitions for new patients.
  • Maintain accurate documentation of contacts, referrals, and marketing activities.
  • Participate in community events and networking opportunities to promote hospice services.
  • Ensure compliance with all state, federal, and organizational regulations. Qualifications
  • Minimum of 1 year of healthcare marketing experience (hospice or home health preferred).
  • Strong knowledge of hospice philosophy and services.
  • Excellent communication, presentation, and interpersonal skills.
  • Ability to work independently and manage multiple priorities.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Proficiency in Microsoft Office and CRM systems.
