Job Description

Performs minimally complex social service work. Reviews Texas Medicaid & Healthcare Partnership (TMHP) internet portal for new and pending assessments. Responsible for conducting screening and evaluation of person(s) seeking admission into a Medicaid-certified nursing facility. Conducts face-to-face evaluation of said individuals to assess need for specialized services, alternate placement options for the individual, and the need for care in a nursing facility or other setting, such as an intermediate care facility for individuals with an intellectual disability or related condition(s). Works under moderate supervision with latitude for the use of initiative and independent judgment. Serves as the point of contact for individuals (or LAR) seeking IDD services. Functions as a liaison between clients (or LAR), internal/external providers, State Mental Health Facilities (SMHF), State Supported Living Centers (SSLC), and Nursing Facilities. Ensures the coordination of services to the persons served, particularly between Nursing Facilities and services within the DADS and DSHS system, to include joint discharge planning, participation in treatment plan reviews and revisions during the transition phase. Maintains documentation and assesses, evaluates and refers clients to needed services. Works under close supervision with minimal latitude for the use of initiative and independent Examples of work performed
  • Ensures the coordination of services to the persons served by providing information regarding center and other community services refers to community resources and when appropriate facilitates the admission process.
  • Will facilitate and ensure communication of information between the Center and state/local facilities.
  • Gathers historical, social, functional, psychiatric, developmental or any other information from the individual (or LAR) seeking IDD services in order to assist Psychologist in determining eligibility
  • Interviews client (or LAR) to gather information used in determining client eligibility for financial, medical, and/or social services.
  • Facilitates Service Coordination of services and supports to individuals in a NF
  • Facilitates the development of the Individual Service Plan and Revisions as needed.
  • Explains program benefits and requirements to client (or LAR).
  • Coordinates Placements/Enrollments into Medicaid Waiver Programs from a Nursing Facility, State Supported Living Center, State Mental Health Facility or ICF/MR Facilities to Community.
  • Screens and completes Admission Applications for Placements to State Supported Living Centers for individuals Residing Nursing Facilities when appropriate.
  • Completes Court Commitment Applications and participates in Court Commitment Hearings.
  • Facilitates visits to community programs when appropriate and addresses concerns about community living.
  • Ensures that the individual receives the educational and informational activities.
  • Describes other community services available to the client and makes referrals as needed.
