Under Direct supervision, performs paraprofessional work in providing care, counseling, and other services to substance abuse clients; and related or other work as required in accordance with Rule 3, Section 3 of the Civil Service Rules. This case manager position is for candidates with at least 2-3 years of case management experience working with diverse and marginalized adult populations. This employee may be assigned to one or more of the following: provide client care and counseling in an outpatient setting, provide case management to mild to moderate substance use clientele. Provide group counseling for emotional, drug, alcohol, or other mental health problems; assist in managing and assisting clients with social, emotional, educational, economic, and other resources/needs.
TYPICAL DUTIES
(will include, but not be limited to ):
- Provides direct care to clients in a community-based full-service partnership mental health outpatient clinic. Helps to evaluate progress; records clinical notes on patient in an electronic health record (EHR) software system.
- Assists professional staff in developing and modifying treatment plans; implements treatment plans.
- Provides case management to mentally ill adults and older adults under the supervision of an LCSW or experienced case manager; may provide counseling, money management, or other related services.
- Arranges for and provides transportation for clients.
- Provides basic client counseling in groups.
- Performs administrative duties as required.
EDUCATION BA
degree must be in Psychology, Social Work, or related Social Science. Certified as Drug Treatment Counselor Possession of a valid California Driver's License is required.
KNOWLEDGE & ABILITY
Must know basic concepts of mental illness, with focus on substance abuse; techniques of dealing with the physical, emotional, and social needs of substance using individuals or substance abusers; clinical terminology. Must be able to learn to develop and maintain the confidence and cooperation of clients and their families. Learn to recognize symptoms of mental or emotional disorders and substance abuse. Able to analyze situations accurately and take appropriate action. Able to understand and follow oral and written instructions. Able to express ideas clearly and concisely both orally and in writing, and to establish and maintain effective working relationships.
