Position Summary:
This position provides support, therapy, and case management to individuals struggling with addiction. Their primary goal is to assess patient needs, develop personalized treatment plans, and facilitate individual or group therapy to prevent relapse and guide clients toward long-term recovery.
Essential Job Functions:
Interview patients, review records, and confer with other professionals to evaluate condition of patients. Formulate treatment plan for treatment and rehabilitation of patients. Provide counseling to patients individually and in group sessions to assist patients in their journey towards recovery. Counsel family members to assist family in dealing with and providing support for patients. Counsel family members individually or in group sessions regarding the impact of addiction in their lives. Participate in utilization review activities as required. Maintain contact on regular basis with referral sources. Maintain patient files and keep prepared for site visits. Develop discharge/transition plans and complete discharge summaries timely. Coordinate and schedule referrals for program placement. Monitor condition of patients to evaluate success and adapt treatment plans. Prepare and maintain reports and case histories. Formulate and conduct programs to promote prevention of alcohol and drug abuse. Understand and implement all TAP 21 Addiction Counseling Competencies. Comply with all CKF policies and procedures and related clinical licensure standards. Other duties as assigned.
