Job Description

CoveCare Center (Formerly known as Putnam Family & Community Services, Inc.,) a private non-profit agency providing substance use disorder treatment services in Putnam County, NY, has a full-time opening for a Rehab Specialist/ Vocational Counselor . We are seeking an individual that shares our vision of partnering with individuals, families and the community to foster hope, wellness and recovery. The Rehabilitation Specialist/Vocational Counselor provides therapy, rehabilitation, and support services, coordinates rehabilitation readiness activities and services to people with psychiatric disabilities in the PROS program. In addition, The Rehabilitation Specialist Employment Specialist provides job development, short-term job coaching as well as vocational rehabilitation and support services.
Responsibilities:
Effectively collects social history, assesses, diagnoses, and treats all populations using the most current clinical tools and modalities, as well as cultural considerations. Uses current modalities appropriate to the diagnosis, as well as within individual, family, couples, and group therapy. Maintains a 65% productivity rate. Interacts appropriately with referral sources and collateral contacts with the highest professionalism and customer service standards. Participates actively, avoiding distractions, and uses time appropriately. This includes avoiding the use of technology for personal purposes. Facilitates appropriate referrals to internal and external programs, as needed, to provide an appropriate level of services. This includes appropriate documentation of referrals. Completes documentation according to document submission standards of the agency and OMH. Demonstrates an understanding of the need to provide services in the context of generating income. Demonstrates an understanding that generating income leads to continued agency viability, continued job security, and continued improvement of services for clients. Completes progress note documentation effectively to demonstrate the service provided, the purpose of the service, and how the service is related to treatment goals and/or core problems. Progress notes are completed clearly and concisely, the same day as the service provided using collaborative documentation. Completes personalized recovery plans and reviews collaboratively with the interdisciplinary team and in a timely fashion (every 3-6 months) per agency and OMH standards/requirements. The recovery plan and progress notes incorporate core problems and client input and are designed to reduce symptoms and promote recovery. Goals are met and the level of care is changed 90% of the time. 100% of services provided are listed in the active recovery plan. Actively monitors the caseload and makes necessary changes in consultation with the supervisor. This includes seeking coaching on how to increase or decrease caseload, including discharging appropriately and timely as per agency and OMH standards/requirements. Demonstrates resolution of all appointments using sound judgment regarding safety, risk, and liability. Demonstrates working knowledge of hospitalization processes and committal processes. Ensures clear hand- off, communication, and clear documentation. Demonstrates cultural competency, co-occurring disorders competency, and competency in clinical work with all ages. Provides consultation and advocacy for individuals in inpatient, residential and independent living situations, including on-site visits and discharge meetings as needed. Accompanies clients to offsite activities (drives agency vehicles). Other related responsibilities and tasks may be assigned. Assists with program and agency crises
Qualifications:
Bachelors' degree in human services required. Two years' experience in vocational rehabilitation, Master's degree and/or CRC desired. A thorough knowledge of diagnostic and therapeutic techniques used in mental health and the factors that contribute to personality development, and a thorough knowledge of the social factors affecting mental health are required. Must have basic computer literacy and excellent verbal and written skills. An insurable driver's license is required, and the use of one's own car may be needed.
