Job Description
Apple Rehab Rocky Hill is a 100-bed skilled nursing center conveniently located off the Silas Deane Highway in Rocky Hill, CT. Our facility offers private and semi-private rooms spacious enough to accommodate residents' personal furnishings. The longevity of our staff creates a family-like environment, with activities and events for both residents and team members to enjoy—indoors and in our courtyard. Apple Rehab is a family-owned and operated company that treats residents and staff like family. Our senior leadership team is based locally in Avon, CT, ensuring responsive support and a strong connection to the communities we serve. Job Description The Social Work Designee (SSD) supports the psychosocial needs of residents under the direction of the Director of Social Services and in accordance with state and federal regulations. The Social Work Designee assists in implementing care plans, coordinating services, and ensuring residents and families receive appropriate support throughout their stay.
Key Responsibilities:
