Apple Rehab Rocky Hill is a 100-bed skilled nursing center conveniently located off the Silas Deane Highway in Rocky Hill, CT. Job Description The Social Work Designee (SSD) supports the psychosocial needs of residents under the direction of the Director of Social Services and in accordance with state and federal regulations. The Social Work Designee assists in implementing care plans, coordinating services, and ensuring residents and families receive appropriate support throughout their stay.

Key Responsibilities:
