Social Worker

Davis Place, a beautiful 5-star, 190 bed, privately owned skilled nursing facility in Danielson CT, is seeking an experienced Social Worker to join their team of highly skilled professionals. Davis Place is a newly renovated facility with a great managerial support structure and is well known for their outstanding patient care and legendary customer service. Responsibilities include but are not limited to: Develop preliminary/comprehensive assessments for each resident. Report any issues/ changes to the Director of Nursing Services Encourage the resident/family to participate in the developmental process and review of resident's plan of care. Develop a written plan of care for every resident that identifies all social problems and needs of the resident, followed by setting goals to be accomplished for each problem/need identified. Ensure that all social services components of the MDS reports are completed and signed in the most efficient manner. Review nurse's documentation to determine if the care plan in place is being followed properly. Excellent opportunity!
